This presentation will review the treatment plans and medications for patients that have end stage renal disease and may need transplants. The presenter hopes to provide Primary Care Physicians with general knowledge of CKD and provide them with confidence when encountering patients with this disorder and especially those that need or had a transplant.

• Grasp how to approach goals of care
• Recognize when to refer to Transplant
• Recognize when to refer to a surgeon for AV access creation
